    The purpose of this training is to provide you with the information to apply to become a Consulting Hypnotist, Certified by the National Guild of Hypnotists, P.O. Box 308, Merrimack, NH, 03054 USA. The manner you treat the information supplied to you is crucial to your success.

    Basic Requirements

    Registering means you are first applying to the NGH for a Non-Certified Membership. Upon acceptance, you will then be automatically enrolled as a Member to take the Certification Course for a Consulting Hypnotist. This and other Fees and benefits are included in your Total Registration Fee. You must complete all paperwork and Application Forms and return them to your Instructor.

    You must attend all classes as scheduled, complete all assignments on time and participate in all class activities. You must complete all work in the allotted time, successfully pass all oral and written exercises and exams and be able to show evidence of successfully completing all practicum, whether inside or outside of the classroom. You must complete all homework, which includes reading at least one book (supplied) within the first two weeks. No application for Certification will be given until all work is successfully completed. Your first year’s Membership in the NGH will be provided by the Instructor upon successfully completing the class requirements. Upon completion of all requirements, you will receive from the NGH a handsome 11 X 14 Certificate of Completion to proudly display.

    In the event that a class must be rescheduled due to unforeseen circumstances, such as, but not limited to, weather or the Instructor’s illness, your class will be rescheduled. This may extend the number of weeks that it will take to complete the Course and it is your responsibility to adjust your schedule in order to attend all classes. No credit or refund will be given for missed or rescheduled class time. In the even that you must drop out of the class, you may ask to retake the missed portions during the next offering, provided you arrange for this before you leave the last class you attend. In the event you miss a scheduled class, you must show evidence of sufficient work to stay current with the class and complete the required number of hours of training. This determination shall be made solely by the Instructor. If you cannot show such evidence, you may elect to be tutored by the Instructor at the additional cost of €80 per hour. Failure to complete all requirements will prohibit Certification until such requirements are met. You must supply a reliable telephone number, email address and physical address on your Application. Post Office Box numbers are not acceptable.

    The information presented in this Course is highly prized and you should treat it accordingly. This information is the foundation upon which you have the distinct opportunity to expand an existing career or use as the basis for a new and exciting career.

    All materials used in this class are copyrighted and may not be used by or shared with anyone that is not enrolled in this class. Recording devices of all types are prohibited and the use of such will be grounds for dismissal without refund or recourse. This will be strictly enforced.

    Martin Kiely Hypnosis Centre Electronic Signature Policy

    It is the policy of the Martin Kiely Hypnosis Centre to accept electronic signatures as defined within this policy for author validation of documentation, content accuracy and completeness with all the associated ethical, business, and legal implications. This process operates within a secured infrastructure, ensuring integrity of process and minimizing risk of unauthorized activity in the design, use, and access of the electronic health records.

    Purpose:

    To establish a foundation for technical and human interaction policy and procedure decisions to guide legal and compliant electronic signature processes. To improve signature legibility, facilitate the use of electronic signatures for client records generated during hypnosis sessions, validate information accuracy and completeness, verify the identification and appropriateness of electronic record authors, and support nonrepudiation.

    Scope:

    Electronic signature, attestation, and authorship are referred to in this document as e-signature. Individuals authorized to affix an electronic signature to a documentation or record shall be limited to individuals with defined privileges.

    Policy Statement:

    Electronic signature is used for records as a means of attestation of electronic record entries, transcribed documents, and computer-generated documents. Properly executed electronic signatures are considered legally binding as a means to identify the author of record entries, confirm content accuracy and completeness as intended by the author, and to ensure e-signature integrity is maintained for the life of the electronic record.

    Types of E-Signatures:

    The policy delineates the types of e-signature functionality acceptable for use in the Martin Kiely Hypnosis Centre and the method of organization approval of each type prior to initial use.

    A properly executed electronic process signifying an approval of an entry or document content presented in electronic format may encompass a broad gamut of technologies and methodologies, ranging from an “I agree” button in a click-through agreement, to an electronic tablet that accepts a handwritten digitized signature, to a digital signature cryptographically tied to a digital ID or certificate.

    Acceptable and approved functional types may include:

    Biometric:

    Use of biological data, such as fingerprints, handprints, retinal scans, and pen strokes, to authenticate an individual.

    Digital signature:

    A cryptographic signature (a digital key) that authenticates the user, provides nonrepudiation, and ensures message integrity. This is the strongest signature because it protects the signature by a type of “tamper-proof seal” that breaks if the message content were to be altered.

    Digitized signature:

    An electronic representation of a handwritten signature. The image of a handwritten signature may be created and saved using various methods, such as using a signature pad, scanning a wet signature, or digital photography. The signature may be “captured” in real time (at the time the user applies the signature), or a saved image captured at the point of normal business operations may be imported. The digitized signature is useful for client signatures that must be collected for consent, application forms or any other type of electronic form requiring client signature.

    Cork Course Tuition and Fee Information

    REFUND POLICY

    Upon written application for cancellation of a registration agreement delivered to Martin Kiely Hypnosis Centre either in person or by registered post, within 3 working days after, such an agreement is signed and up until the date upon which the course is scheduled to start in which the student is enrolled, the student shall be refunded all monies paid. Canceling registration after classes were scheduled to begin, WHETHER THE ENROLLEE WAS IN ATTENDANCE will permit a refund of tuition only according to the schedule outlined below. The minimum refund allowable per student shall be based upon the following formula for a refund.

    “Percentage of Attendance Time” to the total time of the course:

    • Up to 5% – 100% refundable
    • Up to 10% – 90% refundable
    • Up to 25% – 75% refundable
    • Up to 50% – 50% refundable
    • Over 50% – No refund

    NOTE: All course materials remain the property of Martin Kiely Hypnosis Centre and must be returned should a student cancel registration at any stage. No refund will be issued until all course materials are returned and received by Martin Kiely Hypnosis Centre.
